Filing an insurance claim
Follow the instructions below to help ensure success with your insurance claim.

Complete these 3 easy steps before submitting your claim:
- Obtain a prescription from your doctor: Most insurance companies will require a prescription from your doctor listing the product name and prescription codes (See below).
- Obtain itemized receipts: Your insurance will need to see that you purchased a specific Casual Recovery product for a specific amount. Not to worry — you can use the receipt we email you for this part.
- Get your claim form: To submit an insurance claim you will need to fill out your provider's claim form. We are unable to tell you exactly where to find it, but try logging in to your provider's website and searching there.
